Internal Carotid Artery Occlusion caused by Craniopharyngioma in a Childhood - Case Report -

Abstract
The craniopharyngioma usually presents with endocrinologic dysfunction(diabetes insipidus, hypopituitarism etc.), visual disturbance, increased intracranial pressure, and hypothalamic dysfunction. Internal carotid artery(ICA) occlusion by craniopharyngioma is extremely rare. The purpose of this case is to report a very rare case of craniopharyngioma causing ICA occlusion. A 15-months-old female who presented with seizure, vomiting, mental change visited emergency room. Exact evaluation of visual function was impossible due to drowsy consciousness and incooperation. Brain magnetic resonance image and angiography showed acute cerebral infarction due to right ICA occlusion by large cystic tumor with calcification in the suprasellar area which was thought to be craniopharyngioma. The patient was operated with emergency surgical removal of suprasellar tumor for the decompression of optic chiasm and ICA. After tumor decompression operation, cerebral infarction and ICA occlusion were not resolved and visual functions were aggravated to blindness. Considering the deterioration of vision and no recovery of ICA occlusion after radical tumor removal, we suggest alternative therapeutic approaches such as staged operation, bleomycin therapy, and tumor removal with revascularization surgery.
